Why has my blood sugar not gone down? I have been on insulin and januvia as well as metformin and my blood sugar has not been going down.Reply

Perhaps you are not taking enough insulin? Do you know what the ratio of insulin to carbs you require? This is such an individual thing, and takes a little while to figure out, it requires quite a bit of testing, and adjustments. Prior to being on byetta, I was taking humalog and lantus, and over time, my insulin requirements were going up, as I assume I was becoming more insulin resistant. Since being on byetta now for over two years, I no longer take any short acting insulin, only lantus at night, 15 units (I used to take about 50 units).Reply

As to how much insulin, the doctor can give you a chart that indicates how much insulin you should take, based on how many carbohydrates you will eat and your glucose level at the time of eating. For example: if I am going to eat thirty carbohydrates, I will need three units of insulin if my glucose level is under 140, if over then I add one unit, taking a total of three units of insulin. But this is for me, your doctor should give you the guidelines for you.

Being on levemir is a basal insulin, and it is possible you are not taking enough insulin. In my experience, your doctor can only give you guidelines, but it may not necessarily work for you. Have you had any diabetes education? A diabetic educator can go over with you how to increase or modify your insulin dosages. What works for one person may not necessarily work for you. I read a very helpful book, called "Think Like a Pancreas" by Gary Shreiner, it was very helpful when I was solely on insulin for treating my diabetes.Reply

I'd call your dr to see how he/she wants you to increase your Levemir. This is not a short acting insulin that you take according to the carbs you eat but a long acting insulin that you take usually at bedtime. As Teri says you may need to increase your insulin but do so with your drs guidance. Some meds require time to start working but usually if insulin isn't working after a few days an increase is needed. Be careful with your eating though, you can gain alot of weight if you don't cut down on your calorie intake since insulin is a growth hormone. Good luck and let us know how you are doing.

I have been on insulin for a long time(Novolog 26-20-26 and Levemir 90), but following removal of my thryoid I am having very poor control.
You sound very knowledgable. So I thought you might have a comment.
My sugars are going up and down like a roller coaster. During the night they are spiking(sometimes as high as 400) and my morning readings are 150 to 250. I am not getting any sleep because it is keeping me awake.
Do you have any advice?

I don't have any experience with the types of insulin you are on, my suggestion would be to revisit with your endocrinologist, about both your thyroid problem and poor response to insulin.
The short acting insulin I was on was humalog, and the long acting insulin, lantus. Do you know your I:C ratio? Or how much insulin you need for an amount of carbohydrates?
I'm sorry I'm not much help, maybe someone else can chime in.
