Blood Sugar & HbA1c levels

The concern about targeting normal values for blood glucose and A1C in people with diabetes has always been the issue of provoking hypoglycemia if aiming for normal values. Recently, there's been a second concern identified: In patients with T2DM plus cardiovascular disease (or cardiovascular risk), aiming for lower values of A1C (below 6) increased the risk of death compared to aiming for higher values (between 7 & 8). See
http://www.healthcentral.com/diabetes/c/110/30452/accord-advance
for more info. This result was absolutely surprising, and is still unclear as to why aiming for normal A1C would increase the risk of dying. The deaths weren't from hypoglycemia, BTW.

In a few very highly-motivated and very well-educated people, it is indeed reasonable to aim for normal values of BG and A1C -- if they are willing to do zillions of BG tests, and are willing to put up with the potential of severe hypoglycemia.

But I don't think the average person is that motivated, nor that educated.

Actually, after I posted it, I think I'm so fool. Why do I have to worry about that if I really love him.
I myself could get it with a bad nutrition style, and so do my children. They might will not get it if I take a good care of them :)
Thank you anyway!

Welcome.

Glad to hear that it is working so well for you so far. As to when to take it, what did your dr say about when you should take it? Mine said to start taking it in the am but I'm not sure if it makes a difference as to when you take it just as long as you take it the same time every day. The only thing I could think of as to why not to take it in the evening is maybe less effectiveness during the day when you are awake and you might eat more. Other than that I can't say am is better than pm or visa versa.

Call you dr and let us know what he says. Very good question Vicki.

hi geri,
my numbers arent too bad attall really. theyre quite good infact.
im being careful with my carbs and that seems to be helping.
i know the weight loss is a side effect and will differ from one person to the next, but its a side effect i think we all want to make the most of if we cant, dont we?! lol.
and yes, while im eager for victoza to be helping with my bg levels and everything else, losing a few pounds/inches is just as important.
we shall see how it goes, i do hope it works. today has been my first day on 1.8, so heres hoping it will suit me and i can carry on on it. i will let you know in a week or so time how im doing.. :-)
thanks for replying,
kind regards

I am type 2 and was on metformin 2000 mg a day for years with great results. Fasting BS in the 110's to 125's. Had a kidney stone removed and creatine levels escalated very high. They are now back to almost normal, 1.6 creatine level, but. DR took me off of Metformin. I was on Glyburide and gained weight, went off that on to Januvia and BS numbers went up. Now I'm on Januvia and Actos, BS numbers are still high but I continue to gain weight, my hips hurt constantly, and my hands tingle sporadically. I'm not sure my DR knows what to do. Just keeps switching drugs. Any suggestions?

I certainly understand the Frio being more convenient as it is MUCH smaller and easier to store. But as you said, if it gets just a little too warm outside, then it just does not work as desired. On the other hand, the thermos apparently works regardless of the outside temp.

So with temps in the 90s you were getting approximately 24 hours with ice, and 12 hours with ice cold water - either one sounds like a decent solution for extended camping....as long as your cooler still has ice to melt! ;-)

Thanks for the info on some real-world testing of cold water as a cooling source for the thermos solution.

Christsj: I think diabetes is such a complicated disease that figuring out which combination of meds and diet works for each individual can be very difficult. It's possible that your doctor will suggest trying another combination of meds with the Byetta--for instance, metformin or even insulin (that's ok!). How long were you on Byetta before it stopped working well? And could your diet (especially in the evening) have changed enough over time that it is affecting your numbers? Has your exercise schedule changed?

I do believe you can improve the numbers, but it may take some experimenting. Geri often suggests keeping a food/exercise/BG diary as a helpful way to see if you can pinpoint any problems. Timing of your meds is another issue that affects some people. And of course, as time goes on, diabetes can worsen, beta cells can wear out, and you can require more medications or a more rigid diet.

Also, did you mean that you stopped getting the appetite suppression with Byetta? If so, have you tried extending the time between injecting and eating? Or the injection site? If you haven't already, you might read Byetta Basics for some tips. The link can be found at the bottom of the Start page under Recommended reading.

Oh, and do your numbers get significantly better throughout the day? Is it only that fasting BG that is so high or all the time?

Paid Study Opp. Type II diabetics who use Byetta in the SF Bay

Interface Analysis Associates, a San Francisco Bay Area consumer research firm, is conducting a study involving persons with diabetes. Specifically, we are looking for people with Type II diabetes who use the drug Byetta and inject with a pre-filled pen. The goal of the study is to help improve the design and usability of a drug delivery device.

Our next study will last 30 minutes, and pays $60 CASH. The study will take place in our Cupertino, CA research facility on Monday, August 2nd.

Our study is non-invasive; you don't actually take any drug or endure any injection. You simply give feedback on instructions and device operation so that they are made easy to use when they come to market. This is a great opportunity for you to have an impact on the drug delivery systems intended for the diabetes community.

If you or someone you know is interested in participating in this study, please contact us at (408) 834-8443 or via email at iaa_research@yahoo.com. Please refer to the “Byetta Study” and leave your name and day-time number for someone to contact you to discuss your participation.

It may be that Byetta is just not the right medication for you and perhaps you might need insulin to get you under control. Your doctor should find out if you produce any insulin naturally or if it just isn't enough.

It sounds like you gave Byetta a good try and have been doing what you should as far as diet and exercise but I wonder if perhaps you are consuming too many carbs a day? It sounds like you can cut them down some and maybe your numbers will improve.

Having diabetes is frustrating for all or most of us and it is a battle we have to keep fighting. Good luck.
